BayView Healthcare employees were tested for COVID-19 on May 2 and May 3, 2021. All test results have been received and are negative.

Visitations with residents continue in accordance with the Governor’s mandated Emergency Order. BayView Healthcare Update on COVID 19 – April 29, 2021

BayView Healthcare employees were tested for COVID-19 on April 25 and April 26, 2021. All test results have been received and are negative. Residents at the Samantha Wilson Care Center who gave consent were tested on April 22.  All test results have been received and are negative.

BayView Website Update – April 9, 2021

It was previously reported on Thursday, April 8th one employee tested positive for COVID-19. Another employee test result was received late on the same day and it was positive. These employees have been instructed to follow CDC isolation recommendations and return-to-work protocols. Mandatory COVID-19 testing for all employees is being done on Friday, April 9th. Any test results received and are positive will be reported. All employees will be tested again on Sunday, April 11th and Monday, April 12th and any positive test results received will be reported.

Residents who gave consent at the Samantha Wilson Care Center were tested for COVID-19 on Thursday, April 8th. All test results have been received and are negative. Residents from the Samantha Wilson Care Center who give consent will be tested next week.

BayView Website Update – April 8, 2021

On Thursday, April 8th an employee tested positive for COVID-19. This employee has been instructed to follow CDC isolation recommendations and return-to-work protocols.

Due to the positive COVID-19 test result that had been reported,  employee outbreak testing will commence on Thursday, April 8th and Friday, April 9th and on Sunday, April 11th and Monday, April 12th. Any test results received and are positive will be reported.

Residents who gave consent at the Samantha Wilson Care Center were tested on Thursday, April 8th.  Residents who give consent will be tested again one day next week. Any test results received and are positive will be reported.

In keeping with State mandated COVID-19 testing requirements, all employees were tested on February 28 and March 1st.      All tests are negative.  In accordance with CMS guidelines, we are pleased to report we have had no outbreak of COVID-19 among residents in the facilities for over 35 days and, as a result, are not required to test at this time.

BayView Healthcare will be offering the second series of the Pfizer vaccine to all remaining residents who give consent and remaining employees at the Pavilion Assisted Living on March 3rd – 10:00 a.m. until 11:00 a.m. Walgreen clinicians will administer the vaccines.

In keeping with State mandated COVID-19 testing requirements, all employees were tested on February 21 and 22. All tests are negative. In accordance with CMS guidelines, we are pleased to report we have had no outbreak of COVID-19 among residents in the facilities for over 28 days and, as a result,  are not required to test residents at this time.

BayView Healthcare completed its second series of the COVID-19 Pfizer vaccine to all Samantha Wilson Care Center residents who gave consent, employees and major vendors on Friday February 19th by clinicians from CVS Pharmacy.  The second series of the Pfizer vaccine will be administered to all remaining residents who give consent and remaining employees at the Pavilion Assisted Living on March 3rd – 10:00 a.m. until 11:00 a.m. Walgreen clinicians will administer these vaccines.

BayView Healthcare is pleased to announce the second series of the COVID-19 Pfizer vaccine will be made available to the Samantha Wilson Care Center residents who give consent and all employees and major vendors on Friday, January 29th starting at 11:00 a.m. at the main entrance area to the campus. Vaccines will be administered by clinicians from CVS Pharmacy.

Residents who gave consent at the Samantha Wilson Center were tested for COVID-19 on January 20th and January 25th. All test results are negative with no test results pending.

In keeping with State mandated COVID-19 testing requirements, all employees were tested for COVID-19 on January 21st and January 22nd and again on January 24th and January 25th. All test results are negative with one result pending and will be reported if positive.

Visitations with residents continue in accordance with the Governor’s mandated Emergency Order. Clinicians from Walgreens administered the first series of the vaccine program. In keeping with State mandated COVID-19 testing requirements, all employees were rapid tested for COVID-19 on January 17 and January 18. On January 17 one positive employee COVID-19 test result was received. This employee is instructed to follow CDC isolation recommendations and return-to-work protocols. Subsequent employee test results will be reported if positive.

We are happy to remind our community the Pavilion Assisted Living COVID-19 Pfizer vaccine will be made available to its residents who give consent and remaining employees on January 20th from 10 am until 2 pm. Clinicians from Walgreens will administer the first series of the vaccine program.

BayView Healthcare is pleased to announce it launched the COVID-19 vaccination program to residents who gave consent and most employees on Friday, January 8th. Information is forthcoming on the second series of the vaccination. We congratulated and celebrated all staff who participated by awarding special tee shirts, lunch and a chance to win on our “wheel of fortune” spinning wheel. The festivities were in support of providing a safer environment for our residents and employees.

BayView COVID-19 Vaccine Announcement – January 6 2021

BayView is happy to announce the first dose of the COVID-19 Pfizer vaccine will be made available to the Samantha Wilson Care Center residents who give consent and all employees and major vendors on Friday, January 8th at 10:30 a.m. Clinicians from CVS Pharmacy will administer the program.

Due to the COVID-19 positivity rate exceeding 10% in St. Johns County, all BayView employees are mandated to be tested twice weekly. Employees were tested for COVID-19 on Sunday, December 13 and Monday, December 14. We received notification on Wednesday, December 16, two employees test results were positive. BayView Update on COVID-19 – November 2, 2020

In keeping with CMS notification requirements, we received notification on Saturday, October 31st that one employee reported symptoms for COVID 19 and, today, November 2nd, two additional employees reported COVID symptoms. All employees are pending test results and are isolating. No employee will return to their schedules until negative results have been received and symptoms have been resolved.

As mandated by the State, employees will be tested for COVID-19 on Tuesday, November 3rd and Wednesday, November 4th. Results of the testing will be reported when received.

Governor DeSantis’ Emergency Order – DEM 20-011
Update on Visitations

BayView Healthcare has received notification that, under Governor DeSantis’ direction, the Department of Emergency Management has issued Emergency Order 20-011 that includes updates to Compassionate Care and General Visitation rules. A complete copy of the Emergency Order 20-011 may be found on www.flgov.com-covid-19-executiveorders/

The executive order extends the definition of a Compassionate Care Visitor to be able to provide support, including emotional support, to help residents deal with difficult transition or loss, upsetting events, end-of-life situations, significant changes (such as recent admission to the facility), need for assistance, cueing or encouraging with eating or drinking, or emotional distress or decline.

General Visitation will now include outdoor visitation if the facility has a new onset of resident or staff COVID-19 cases and allow anyone under the age of 18 for general visits.

In keeping with Emergency Order 20-011 guidelines, compassionate care, essential care, and general visitation will continue in accordance with the newly mandated Emergency Order 20-011. The following is a copy of BayView Healthcare’s revised general visitation policy:

As outlined in the Governor’s Executive Order DEM ORDER NO. 20-011 the following criterias have been put in place for general outdoor visitations. The guidelines meet state and federal regulations regarding general visitation while minimizing the risk of spreading communicable disease to promote the health and safety of our residents and staff.

GENERAL VISITORS

General Implementation

General visitors will be permitted when the facility meets the following criteria:

1. Sufficient staff to support management of visitors
2. Adequate PPE for all staff
3. Adequate cleaning and disinfecting supplies
4. Adequate capacity at referral hospitals for the facility

General visitors must:

1. Wear a face mask and perform proper hand hygiene
2. Sign a consent form noting the understanding of the facility’s visitation and infection prevention and control policies
3. Call the facility to schedule an appointment for visitation
4. No more than two (2) general visitors per resident are permitted at one time
5. Visit in a designated outdoor area
6. Maintain social distance of at least six feet with staff and residents, limit movement in the facility
7. Meet visitor screening criteria
8. Visitors must immediately inform the facility if they develop a fever or symptoms consistent with COVID-19 within 14 days of a visit or test COVID-19 positive, or has been in close contact with persons infected with COVID-19
9. Follow the facilities policies and procedures for visitation

Individuals seeking entry into the facility will not be allowed to enter if they meet any of the criteria below:

1. Any person infected with COVID-19
2. Any person with signs and symptoms of COVID-19 within the last fourteen (14) days
3. Any person who has been in contact with a person known to be COVID-19 positive
4. Any person who has traveled on an airplane or cruise ship within the last fourteen (14) days
5. Any person who refuses to wear PPE per CDC guidelines.

Any persons denied entry into the facility by the interdisciplinary team (Director of nursing/designee, Infection Preventionist, Social Services) may file an appeal with the administrator.

Before allowing general visitation, the facility shall:

1. Prohibit visitation if the resident receiving general visitors is quarantined, positive for COVID-19 or symptomatic for COVID 19
2. Screen all visitors to prevent possible introduction of COVID-19
3. The Samantha Wilson Care Center shall limit the number of visitors in the building to no more than four (4) visitors at one time to safely screen and monitor visitation. No more than two (2) residents may have visitors at one time with a maximum of four (4) visitors in the facility at any time. Residents may not schedule more than one visit per day.
4. The Pavilion Assisted Living shall limit the number of visitors in the building to no more than four (4) visitors at one time to safely screen and monitor visitation. No more than two (2) residents may have visitors at one time with a maximum number of four (4) visitor in the facility at one any time. Residents may not schedule more than one visit per day.
5. Visiting hours will be Monday, Wednesday, Friday, Saturday and Sunday from 10:00 a.m. until noon and 2:00 p.m. until 6:00 p.m.
6. Visits will be no longer than 30 minutes in length
7. Visitation areas are located in designated areas on the campus and visitors will be in-serviced of areas at time
8. Visitation for the Samantha Wilson Care Center may be scheduled by calling 904-584-1847, Monday through Friday, 9:00 a.m. to 5:00 p.m. Visitation for the Pavilion may be scheduled by calling 904-584-1669, Monday-Friday from 9:00 a.m.-5:00 p.m.
9. Clean and disinfect visitor spaces between visitors
10. Maintain documentation of individuals entering the facility

BayView will keep its community informed of any policy updates or changes regarding
visitations to our facilities.

As reported earlier today, we received notice that one staff member at the Samantha Wilson Care Center tested positive for the COVID-19. All remaining test results for staff have been received and are negative. Residents who give consent will be tested on Saturday, October 24th.

General Visitation Cancellation Notice Effective Friday, October 23, 2020

We received results this morning that an employee at the Samantha Wilson Care Center has tested positive for COVID-19. Under the Governor’s executive order on general visitation guidelines, general visitations are prohibited if any resident tests positive for COVID-19 or a staff member who was in the facility in the ten days prior tests positive for COVID-19. All employees were tested on Tuesday, October 20th and Wednesday, October 21st. All residents who gives consent at the Samantha Wilson Care Center will be tested on Saturday, October 24th. We will continue to provide information of the results once received.

In accordance with the Governor’s Executive Order, the facility must have no new facility-onset of resident or staff COVID-19 cases in the previous fourteen (14) days. As outlined in the regulation it would not be possible for us to open for general visitations until November 7, 2020.

There are no changes to the Essential and Compassionate Care Giver visits schedule already in place.

BayView Healthcare is excited to celebrate some welcoming news to our community. Test results for residents who gave consent and all staff came back negative. We are pleased to announce we have met the criteria to allow general visitation to our residents Samantha Wilson Care Center and the Pavilion Assisted Living.

Visitation appointments for the Samantha Wilson Care Center may be scheduled by calling 904-584-1847, Monday through Friday, 9:00 a.m. to 5:00 p.m.

Visitation appointments for the Pavilion Assisted Living may be scheduled by calling 904-584-1669, Monday through Friday from 9:00 a.m.-5:00 p.m.

The following are the guidelines for safe and limited re-opening of our facilities for general visitations:

As outlined in the Governor’s Executive Order DEM ORDER NO. 20-009, the following criterias have been put in place for general visitations. The guidelines meet State and Federal regulations regarding general visitation while minimizing the risk of spreading communicable disease to promote the health and safety of our residents and staff.

Guideline implementation

General visitors other than essential caregivers or compassionate care visitors, will be permitted when the facility meets the following criteria:

1. Other than a dedicated wing or unit that accepts COVID-19 cases from the community, the facility must have no new facility-onset of resident COVID-19 cases in the previous fourteen (14) days;
2. The facility must have fourteen (14) days with no new facility-onset of staff COVID-19 cases where a positive staff person was in the facility in the ten (10) days prior to the positive test;
3. Sufficient staff to support management of visitors;
4. Adequate PPE for all staff;
5. Adequate cleaning and disinfecting supplies; and,
6. Adequate capacity at referral hospitals for the facility.

General visitors must:

1. Must be of eighteen (18) years or older;
2. Each resident may designate five (5) visitors;
3. Wear facility provided PPE and perform proper hand hygiene;
4. Sign a consent form noting the understanding of the facility’s visitation and infection prevention and control policies;
5. Visitation for the Samantha Wilson Care Center may be scheduled by calling 904-584-1847, Monday through Friday, 9:00 a.m. to 5:00 p.m. Visitation for the Pavilion may be scheduled by calling 904-584-1669, Monday-Friday from 9:00 a.m.-5:00 p.m.;
6. No more than two visitors per resident are permitted at one time;
7. Visits will be held outdoors in designated areas or in a facility designated area;
8. Maintain social distance of at least six feet with staff and residents, limit movement in the facility;
9. Meet visitor screening criteria;
10. Visitors must immediately inform the facility if they develop a fever or symptoms consistent with COVID-19 within 14 days of a visit or test COVID-19 positive, or has been in close contact with persons infected with COVID-19; and,
11. Follow the facilities policies and procedures for visitation.
12. Adherence to – no open-toe shoes or personnel belongings permitted into the facility with the exception of cell phone and car keys.

9. Maintain documentation of individuals entering the facility.

Bayview will immediately cease general visitation if a resident tests positive for COVID-19, or is exhibiting symptoms indicating that he or she is presumptively positive for COVID-19, or a staff member who was in the facility in the ten (10) days prior tests positive for COVID-19.

BayView will notify and inform residents/representatives of any policy updates or changes regarding visitation.

There are no changes to the appointment schedule for the Essential and Compassionate Care Givers already in place.

We are pleased to report there are no new COVID-19 cases at the Samantha Wilson Care Center and the Pavilion Assisted Living. All residents who gave consent at the Samantha Wilson Care Center were tested on Wednesday, October 14th and are negative with one pending as of this date. Employees at both facilities were tested on Tuesday, October 13th and Wednesday, October 14th and testing results are all negative.

In preparation of when BayView Healthcare may meet the Governor’s criteria in allowing general visitations, the following is a policy developed for the safe and limited re-opening of our facilities for visitors:

As outlined in the Governor’s Executive Order DEM ORDER NO. 20-009, the following criterias have been put in place for general visitations. The guidelines meet State and Federal regulations regarding general visitation while minimizing the risk of spreading communicable disease to promote the health and safety of our residents and staff.

Guideline implementation:

General visitors other than essential caregivers or compassionate care visitors, will be permitted when the facility meets the following criteria:

1. Other than a dedicated wing or unit that accepts COVID-19 cases from the community, the facility must have no new     facility-onset of resident COVID-19 cases in the previous fourteen (14) days;
2. The facility must have fourteen (14) days with no new facility-onset of staff COVID-19 cases where a positive staff person was in the facility in the ten (10) days prior to the positive test;
3. Sufficient staff to support management of visitors;
4. Adequate PPE for all staff;
5. Adequate cleaning and disinfecting supplies; and,
6. Adequate capacity at referral hospitals for the facility.

General visitors must:

1. Must be of eighteen (18) years or older;
2. Each resident may designate five (5) visitors;
3. Wear facility-provided PPE and perform proper hand hygiene;
4. Sign a consent form noting the understanding of the facility’s visitation and infection prevention and control policies;
5. Visitation for the Samantha Wilson Care Center may be scheduled by calling 904-584-1847, Monday through Friday, 9:00 a.m. to 5:00 p.m. Visitation for the Pavilion may be scheduled by calling 904-584-1669, Monday-Friday from 9:00 a.m.-5:00 p.m.;
6. No more than two visitors per resident are permitted at one time;
7. Visits will be held outdoors in designated areas or in a facility designated area;
8. Maintain social distance of at least six feet with staff and residents, limit movement in the facility;
9. Meet visitor screening criteria;
10. Visitors must immediately inform the facility if they develop a fever or symptoms consistent with COVID-19 within 14 days of a visit or test COVID-19 positive, or has been in close contact with persons infected with COVID-19;
11. Follow the facilities policies and procedures for visitation; and,
12. Adherence to – no open-toe shoes or personnel belongings permitted into the facility with the exception of cell phone and car keys.

Individuals seeking entry into the facility will not be allowed to enter if they meet any of the criteria below:

1. Any person infected with COVID-19;
2. Any person with signs and symptoms of COVID-19 within the last fourteen (14) days;
3. Any person who has been in contact with a person known to be COVID-19 positive;
4. Any person who has traveled on an airplane or cruise ship within the last fourteen (14) days;
5. Any person who refuses to wear PPE per CDC guidelines;
6. Any person who does not meet the facility visitor screening criteria; and,

Any persons denied entry into the facility by the interdisciplinary team (Director of Nursing/designee, Infection Preventionist, Social Services) may file an appeal with the Administrator.

Before allowing general visitations, the facility shall:

1. Prohibit visitation if the resident receiving general visitors is quarantined, positive for COVID-19 or symptomatic for COVID-19;
2. Screen all visitors to prevent possible introduction of COVID-19;
3. The Samantha Wilson Care Center shall limit the total number of visitors in the building to no more than four (4) visitors at one time to safely screen and monitor visitation;
4. The Pavilion Assisted Living shall limit the total number of visitors in the building to no more than two (2) visitors at one time to safely screen and monitor visitation;
5. Visiting hours will be Monday, Wednesday, Friday, Saturday and Sunday from 10:00 a.m. until noon and 2:00 p.m. until 6:00 p.m.;
6. Visits will be no longer than 30 minutes in length;
7. Visitation areas are located in designated areas on the campus and visitors will be in-serviced of areas at time of visit.
8. Clean and disinfect visitor spaces between visitors; and,
9. Maintain documentation of individuals entering the facility.

Bayview will immediately cease general visitation if a resident tests positive for COVID-19, or is exhibiting symptoms indicating that he or she is presumptively positive for COVID-19, or a staff member who was in the facility in the ten (10) days prior tests positive for COVID-19.

BayView will notify and inform residents/representatives of any policy updates or changes regarding visitation.

If there are no reported positive COVID-19 test results received from the recent round of testing conducted, appointments may be made starting Monday, October 19th. Visitation appointments for the Samantha Wilson Care Center may be scheduled by calling 904-584-1847, Monday through Friday, 9:00 a.m. to 5:00 p.m. Visitation appointments for the Pavilion Assisted Living may be scheduled by calling 904-584-1669, Monday-Friday from 9:00 a.m.-5:00 p.m.

Exceptions to the appointment schedule may be considered on a case-by-case basis as we wish to be as accommodating as possible.

There are no changes to the appointment schedule for the Essential and Compassionate Care Givers already in place.

Our team of caregivers respectfully request all visitors adhere to the policies put in place as it is instrumental to our commitment in keeping our residents and staff safe at all times.

Covid-Update on Website – October 9, 2020

We are pleased to report there are no new COVID-19 cases at the Samantha Wilson Care Center and the Pavilion Assisted Living. All residents who gave consent at the Samantha Wilson Care Center were tested on October 6th and are negative. Employees at both facilities were tested on October 5th and October 6th and testing results are all negative.

Under Governor DeSantis’ Executive Order on general visitation guidelines, we may anticipate general visitation to start on October 17th. This date is subject to change pending on any new developments related to the COVID-19 virus. A general visitation policy is being developed as recommended by the Executive Order and will be made available to all family members and/or responsible parties. Essential Care and Compassion Care visitation continues to go well and is appreciated by the residents and families.

We want to extend our heartfelt appreciation to our staff for their effort and dedication in following our infection control practices in caring for themselves and especially our residents. We will keep the community informed of ways we continue to improve in keeping our residents and staff safe during the COVID-19 event.

COVID-Update on Website – October 2, 2020

All employees at the Samantha Wilson Care Center and the Pavilion Assisted Living were tested for COVID-19 on September 27th and September 28th. All test results have been received and are negative.   Employees will be tested as mandated by the State on October 5th and October 6th.

Residents who gave consent at the Samantha Wilson Care Center were retested for COVID-19 on Wednesday, September 30th. All test results received to date were negative, with the exception of one resident who has tested positive. This resident is asymptomatic and is on isolation protocols. There is one test result pending and the result will be reported when received and if positive.

A training session for qualified Essential Care and Compassionate Care Givers was held on Sunday, September 27th. Visitation schedules for both facilities have been provided to the family members and residents and families have expressed their heartfelt appreciation for the program in place.

We would like to take this opportunity to remind our BayView staff and all caregivers that screening and infection control protocols are in effect for the benefit of protecting our residents. Everyone who enters the facility must abide by the protocols in place (i.e. respond to all questions presented by screeners; proper dress attire and wear appropriate personal protection equipment as requested when visiting and/or treating patients. As reported earlier, on September 16th residents from the Samantha Wilson Care Center who gave consent were retested for COVID-19. On September 18th, there were 3 new positive COVID-19 test results received. These residents were asymptomatic when reported and are in isolation. As of today, two of these residents have developed symptoms. All other residents who were tested are negative. There are no new residents at the Pavilion Assisted Living who have tested positive for COVID-19.

BayView Healthcare Update on Covid-19     September 17, 2020

BayView pledges to keep residents, families and our community informed of updated protocols and infection control practices during the COVID-19 pandemic. We continue to monitor the CDC to help us maintain best practices to keep our community safe.

On September 14, 2020, BayView posted a website published letter to the community regarding Governor Desantis Emergency Order 20-009 with specific guidelines to follow. As indicated in our letter, Compassionate Care Givers and Essential Care Givers may be allowed into the facility once these individuals meet the criteria outlined in the Executive Order. This includes special training in infection control and other protocols outlined in our policy.

Trainings for Compassionate Care Givers and Essential Care Givers will be held on Sunday, September 20th at 2:00 p.m or Monday, September 21st at 6:00 p.m. and should meet at the main entrance to BayView.

On Sunday, September 13 and Monday, September 14, another State mandated COVID-19 testing for all staff was administered and all test results have been received.

Test results for employees at the Pavilion Assisted Living are negative with the exception of two employees who’s test results came back indeterminate.  Both of these employees have been retested and results are pending.

A positive test result was received for one employee at the Samantha Wilson Care Center on September 16th. This person is asymptomatic and is in isolation until they meet the requirements for return-to-work criteria. A total of two employees had exposure to this employee and are now in isolation and are scheduled to be retested. A total of 4 employees at the Samantha Wilson Care Center had test results that were indeterminate. One of these employees has been retested and received a negative result. The remaining 3 employees have been scheduled to be retested. We will continue to keep you informed of pending test results for staff when received.

Currently, there are no new residents at the Samantha Wilson Care Center or the Pavilion Assisted Living who have tested positive for COVID-19 residents who gave consent at the Samantha Wilson Care Center were retested on Wednesday, September 16th with results pending.

BayView Healthcare Visitation with Residents

Dear Valued Residents and Families:

BayView Healthcare understands how important it is to be able to visit with your loved ones. On September 1st, Governor Desantis lifted the visitation restrictions that had been in place to protect residents from COVID-19. In order to keep the safety and well-being of everyone as a top priority, the State’s new policy establishes specific guidelines that must be followed. Emergency Order 20-009 supersedes DEM orders 20-006 and 20-007. The Emergency order can be found at https://www.floridadisaster.org/globalassets/covid19/dem-order-20-009-signed.pdf

Emergency Order 20-009 has created two new categories of visitors:

Compassionate Care Visitors are individuals over the age of 18 who provide emotional support to deal with a difficult transition or loss or end of life. Each resident/representative may designate up to two Compassionate Care Visitors; however, only one of these visitors may be with the resident as stipulated in the plan of care at a time.

Essential Care Givers are individuals over the age of 18 who are given consent by the resident/representative to provide services and or assistance with activities of daily living including bathing, dressing, eating or emotional support. The interdisciplinary team will review all requests to become an Essential Care Giver and will be identified and included in the residents plan of care. Essential Care Givers include persons who provided care before the pandemic and those who request to provide services. Each resident may designate up to two Essential Care Visitors; however, only one of these visitors may be with the resident as stipulated in the plan of care at a time.


Residents may have one visitor at a scheduled time to be agreed upon by the facility and visitor. All visitors must receive training in infection control, hand hygiene, and social distancing. All visitors are required to wear personal protective equipment and follow the facilities policies and procedures for visitation.


Anyone who wishes to become a designated Compassionate Care Visitor or Essential Care Giver, please contact the Samantha Wilson Care Center Department of Social Services at 904-584-1569 or 904-584-1847. You may contact Ms. Theresa Ortiz, Administration, Pavilion Assisted Living at 904-755-6498. A member of the interdisciplinary team will contact you to discuss visitation schedules, care and services provided in the resident’s care plan.

General visitation will resume when the facility has not had any new onset of COVID-19 positive residents or staff in the previous 14 days. Visitation will be prohibited if the resident receiving general visitors is quarantined, positive for COVID-19 or symptomatic for COVID-19. Each resident may designate up to five visitors for general visitation but only two may visit at a time.

The facility will monitor for adherence to infection control, personal protective equipment, hand hygiene and social distancing. All visitors will be required to sign an acknowledgement of completion of training and adherence to the facilities’ policies.


We are currently working to implement appropriate policies to comply with the Emergency Order so you may visit with your loved ones in the safest environment possible. These policies will include but not limited to:
• Visitor screening process and policy
• Visitor PPE (personal protective equipment), infection control, hand hygiene and social distancing trainings
• Process for scheduling appointments for visitation
• Establish number of visits per week by an individual
• Designation and utilization of Essential Caregiver or Compassionate Care Visitor
• Designated visitation locations
• Policies to set maximum number of visitors in the building safely
• Adhere to facility visitation policies

BayView Healthcare Website Update on COVID-19  – September 8, 2020

We have received results from the Florida Department of Health on Monday, September 7th that two additional residents from the Samantha Wilson Care Center have tested positive for COVID-19. One resident was asymptomatic and the other had symptoms. Families have been notified and isolation precautions and increased monitoring have been put in place. There are no test results pending for the residents who gave consent at the Samantha Wilson Care Center.

All test results for residents who gave consent at the Pavilion Assisted Living are negative with no test results pending.

Test results received for staff in both facilities are negative with 41 test results pending and will be reported when the results have been provided.

Please note reporting whether or not a resident is symptomatic is limited to the point in time of reporting.  BayView is not able to report on the medical conditions of residents who have been transferred to other facilities or hospitals.

As mandated by the State, another COVID-19 testing for all staff is being administered today, September 8th and on Wednesday, September 9th. Staff is to report to the main entrance of the facility from 6 a.m. until noon or 1:00 p.m. until 2:30 p.m.

In accordance with the Executive Ordered issued under Governor DeSantis to allow visitation to our facilities, certain key conditions must be followed. BayView is one main building with shared staff, some of whom provide services for residents in both the skilled nursing and assisted living settings (i.e. maintenance, dietary, therapists, etc.) Therefore, there must be no positive COVID-19 cases of any resident or employee or anyone who is asymptomatic and isolated for a minimum of 14 days in both programs.

In accordance with this regulation, at this point in time we will not be allowed for indoor/outdoor visitation until September 22nd. This date could be revised if there are newly reported cases with positive test results. BayView Healthcare Website Update on COVID-19      September 6, 2020

The Florida Department of Health tested residents at the Samantha Wilson Care Center and the Pavilion Assisted Living who gave consent and all staff members on Thursday, September 3rd. It was reported on Saturday, September 5th that all Pavilion test results we have received thus far are negative with 15 test results pending. There are three residents who tested positive at the Samantha Wilson Care Center. All three residents are reported to be asymptomatic. The families have been notified and residents have been moved into isolation. Increased monitoring will be put into place. There is a total of 16 test results pending. Information on test results pending for residents and employees will be provided when received.

In accordance with the Executive Ordered issued under Governor DeSantis to allow visitation to our facilities, certain key conditions must be followed. BayView Healthcare Website Update on COVID-19 – September 2, 2020

As a follow up to the all staff COVID testing completed on August 28, 2020, a positive test result was received for one employee on September 1. This individual is in isolation and will follow the return-to-work infection control protocols.

All 14 residents who tested positive at both facilities remain in isolation and are non-symptomatic.

The Florida Department of Health will be testing consenting residents at both facilities and employees on Thursday, September 3rd starting at 8:30 a.m. lt is mandatory that all staff be tested and should report to the main entrance to the BayView campus at the designated time. We will continue to provide information of the results once received from the Florida Department of Health.

We are pleased to report that on Tuesday, September 1st, an Executive Order was issued under Governor DeSantis’ direction to lift restrictions for visitation to nursing homes and assisted living facilities. The Executive Order was based on recommendations made by the Governor’s task force on the safe reopening of the facilities. One of the key conditions that must exist for a facility to allow indoor and/or outdoor visitation is there must be no resident or staff member who tested positive within the last 14 days. Since we have reported positive COVID-19 cases within both facilities, we currently do not meet the guidelines to offer indoor and/or outdoor visitation. As outlined in the new regulations it would not be possible for us to open prior to September 15, 2020. This date could be revised if there are newly reported cases with positive test results of any resident or staff member or if any resident is on isolation. BayView Healthcare COVID-19 Update August 28-2020

BayView Healthcare has 14 new confirmed positive COVID-19 test results and all persons affected are following isolation protocols. Testing of residents who gave consent at the Samantha Wilson Care Center and the Pavilion Assisted Living was completed by 4:15 p.m. today via the new Point of Care rapid testing. Results were returned for each resident within 15 minutes. We have received results for all residents in both facilities. The vast majority of residents were non symptomatic.


In response to the outbreak, testing of all staff was completed by 2:30 p.m. today. We will provide information once the test results have been received and monitor accordingly. These resident and staff testing measures were taken in compliance with the Centers for Medicare and Medicaid Services (CMS) guidelines.

BayView Healthcare COVID-19 Update August 27, 2020

There have been two confirmed positive COVID 19 test results at BayView Healthcare. As a result, all employees and contractors must be tested today or tomorrow. The test times are set for August 27 from noon until 7:30 p.m. and August 28 from 6 a.m. until noon and 1:00 until 2:30 p.m. Residents at both facilities also will be tested.

BayView Healthcare COVID-19 Update August 22, 2020

As previously reported, all staff were tested for COVID-19 on August 18th and August 19th as mandated by the State of Florida.

At the Pavilion Assisted Living, test results for all staff, with the exception of one, have been received and all are negative.

All but two test results for staff at the Samantha Wilson Care Center have been received. All test results are negative with the exception of two employees who tested positive. Both employees with positive test results are isolating and will not be scheduled to return until the return-to-work protocols have been followed. Two tests were returned inconclusive and rapid tests were completed and are negative.

BayView Healthcare COVID-19 Update – August 20, 2020

The latest round of the State mandated COVID-19 testing for all employees has been completed on August 19th at 2:30 p.m. We will monitor all test results received and will take appropriate action when needed.


In addition, we received results on August 19th that an employee at the Samantha Wilson Care Center has tested positive for COVID-19. On August 17th the employee reported symptoms, has been isolating since that date. This employee will not return to the facility until a negative test result is received.


The State’s executive order extending the visitation prohibition to Florida long-term care and assisted living facilities is still in effect. As a reminder to all of our employees, if you are unable to report to work please call the designated call-in number 904-540-0395. BayView Healthcare COVID-19 Update – August 13, 2020

We are pleased to report there are no new COVID-19 cases at the Samantha Wilson Care Center and the Pavilion Assisted Living among its residents or staff. As of August 12, the two residents who were isolated for precautionary reasons are now off isolation and asymptomatic. The designated COVID unit has been deactivated. We express our deepest gratitude for the commitment and dedication to the staff who met the challenges they faced in providing the best of care to the residents they served.

As reported from the last State mandated COVID-19 test for all staff, the one staff member whose test result came in inconclusive was retested and the test result was negative. The next mandated all-staff testing will take place on Tuesday, August 18th and Wednesday, August 19th.  Times to report are  6 a.m. to noon and 1:30 p.m. until 3:00 p.m.

BayView Healthcare COVID-19 Update – August 8, 2020

We are pleased to report all staff were tested for COVID-19 on August 4 and August 5 as mandated by the State of Florida. Test results have been received for all staff at the Pavilion and are negative. Test results for staff at the Samantha Wilson Care Center have been received and are negative. Two tests were returned inconclusive. One of the staff member was sent for a rapid test which was negative. The other is to schedule a follow-up test and must test negative before returning to the facility. At this time, there are no new COVID-19 cases reported for residents at either the Samantha Wilson Care Center or Pavilion Assisted Living.

As reported previously, residents who had tested positive and residents who were symptomatic have been moved into a dedicated COVID unit in the facility. To date, six residents have been relocated out of the unit into their designated rooms and one additional resident discharged. Eight residents are in isolation.

We remain deeply grateful for our staff and how they are meeting the numerous and complex challenges due to the COVID-19 pandemic. Our employees have demonstrated amazing diligence, courage, and compassion.

BayView Healthcare COVID-19 Update – August 5, 2020

We received results on August 4 that an employee at the Samantha Wilson Care Center has tested positive for COVID-19. On August 2 the employee reported symptoms, has been isolating since that date and last worked on July 28. This employee will not return to the facility until two negative test results are received.

Any previously reported employees who have not met the two negative test protocol and/or CDC guidelines have not returned to work.

As required, mandatory COVID testing for all employees was completed today at approximately 2:30 p.m. We will monitor all test results when received and take appropriate action.

BayView Healthcare Covid-19 Update – August 1, 2020

In keeping with CMS notification requirements, on July 29th we had an employee who reported symptoms and two tests have been scheduled.   This employee is isolating.  On July 31, we had an employee who reported symptoms and had a negative rapid test result on the same day and is still symptomatic. This employee is isolating and a second test has been scheduled.   On the same day, another employee reported symptoms and two tests are to be scheduled.  An employee who reported symptoms earlier, reported on 7/31 to have tested positive.  The employee is in isolation and will not return to work until two negative test results are received.  

As mandated by the State, employees will be tested for the COVID-19 on Tuesday, August 4 and Wednesday, August 5th.  A schedule of times to report will be made available to all employees prior to the testing dates.

BayView Healthcare Covid-19 Update – July 27, 2020

As an infection control measure to help further prevent the spread of the COVID-19 into the facilities, starting Wednesday, July 29th, BayView will not be accepting any personal items into the building from family members, friends and employees.

With the exception of two employees at the Samantha Wilson Care Center, all COVID-19 test results for all employees have been received and are negative. Previously reported, there were two positive employee test results at the Samantha Wilson Care Center.

At the Pavilion Assisted Living, test results for all residents, with the exception of one, have been received and all are negative. The Samantha Wilson Care Center test results have been received for all residents with no new positive test results reported.

This is a reminder that Florida Governor DeSantis has issued an executive order extending the visitation prohibition to Florida long-term care and assisted living facilities for an additional 60 days. The extension currently is in effect until September 5, 2020. We realize this is a hardship on residents and their family members, and we will keep you informed about any changes in this prohibition requirement. BayView Healthcare Covid-19 Update – July 25, 2020

The Centers for Medicare and Medicaid Services (“CMS”) requires us to notify residents, the residents’ responsible parties, and staff regarding any new confirmed COVID-19 cases or new symptom outbreaks in our facilities. This emphasis on transparency is supported by our organization. We are reporting to you here all symptom outbreaks, the results of the Florida Department of Health COVID-19 test results of all consenting residents at the Samantha Wilson Care Center and the Pavilion Assisted Living that was conducted on July 22, 2020 and the State mandated COVID-19 testing results of all staff conducted on July 21 and July 22, as of 5:00 p.m., EDT today.

Consistent with best-practice infection control and prevention procedures associated with COVID-19, all residents who tested positive and residents who are symptomatic have been moved into a dedicated COVID unit in the facility. Designated nurses, certified nursing assistants, therapists, dietary aides and housekeepers have been assigned to provide 24-hour direct care in the unit. Infection control safety measures are in place to avoid cross-contamination of the virus into other areas of the campus. Use of enhanced personal protection equipment such as gowns, shields, masks, gloves, etc., are being used by staff who are assigned in the COVID unit. N-95 masks are being used by all employees in both facilities. Inventory of personal protective equipment and other supplies are updated daily.

At the Samantha Wilson Care Center, a total of 11 positive results have been identified. A total of 4 residents are symptomatic and are under surveillance. At the Pavilion Assisted Living, only two tests results have been received for residents, and they are negative. In addition, a total of 10 negative test results have been reported for Pavilion employees, and 2 Samantha Wilson Care Center employees have tested positive. Positive employees will not be allowed to return to work without meeting enhanced CDC testing requirements.

We received results today regarding an employee at the Samantha Wilson Center who tested positive for COVID-19. The employee’s test was administered on Friday, July 17. This employee was wearing a mask at all times. All BayView contacts with this employee are being traced. Continued enhanced respiratory monitoring, temperature and oxygen saturation checks every four hours are in place. All employees are now wearing N95 masks. The Florida Department of Health plans to retest all consenting residents on Wednesday, July 22nd at 9:00 a.m. All staff will be retested on Tuesday, July 21 and Wednesday, July 22 as mandated by the State.

BayView Healthcare Covid-19 Update – July 17, 2020

As a follow-up to the July 16th website update, the resident at the Samantha Wilson Care Center tested negative again today for the COVID-19. However, a resident that this person was in contact with was rapid tested this morning, tested positive and became symptomatic today. After evaluation by the hospital emergency department, the resident was transferred to a dedicated COVID-19 facility in the Jacksonville area. All other residents and working staff currently are asymptomatic for COVID-19.

The Florida Department of Health plans to retest all consenting residents on Wednesday, July 22nd at 9:00 a.m. All staff will be retested on Tuesday, July 21 and Wednesday, July 22 as mandated by the State.

BayView will continue enhanced screening and monitoring of all residents and staff members including safety and infection control protocols.

BayView Healthcare Covid-19 Update – 7-16-2020

In keeping with BayView’s reporting requirements, we learned this morning that a patient at the Samantha Wilson Care Center who was anticipating to be discharged and required COVID-19 testing for transfer to an assisted living facility, received a positive test result. This patient has never had any symptoms and was retested this morning (with a rapid test) and had a negative COVID-19 result. There are no other new Covid-19 cases to report either at the Samantha Wilson Care Center or the Pavilion Assisted living.

Out of an abundance of caution, all safety and infection prevention protocols (isolation, use of protective equipment, N-95 masks, gowns, face shields, gloves, etc.) are in place to protect the wellness and safety of all residents and staff. Also, the positive patient is scheduled to be retested again tomorrow morning with an anticipated rapid test result on the same day.

Though some of the details are confusing and we are still looking into what happened, it should be noted that there were numerous problems with the reported positive result. Shortly after the test was administered, the testing provider could not find the sample. After finding the sample, we were then notified there was a problem with the reagent used for the test. The sample was sent to a different lab. We first received notice of the positive result with no written verification. After receiving the verbal notice, we received an additional verbal notice that the test had not been completed. After receiving notice that the results had not been received, we received a written confirmation of the positive result. We requested that the sample be retested and were told the lab would not retest the sample. Given these circumstances, we are very grateful that Flagler Hospital was able to provide follow up rapid testing so quickly and are thankful that this test was negative for COVID-19.

The Florida Department of Health will be conducting retesting of all employees and consenting residents in the near future, and we will notify you of the time and date as soon as it is scheduled.

The first round of the State mandated COVID-19 testing for all employees has been completed. Almost all of the results have been received. To date there are no positive test results.

Though we are pleased to report there are no new, confirmed COVID 19 cases at either the Samantha Wilson Care Center or the Pavilion Assisted Living at BayView, we did want to let our residents, their family members and our staff know that we have completed our first round of diagnostic Covid-19 testing for all employees as of today. This testing regime, which requires all employees to be tested approximately every two weeks, will continue for the foreseeable future. Testing resources under this program are provided by the State of Florida.

BayView Healthcare Employee Mandated Covid-19 Test

July 8  through July 9, 2020

The State of Florida has issued a mandate requiring all nursing homes and assisted living facilities staff be tested for the Covid-19 every two weeks. Testing resources will be provided by the State. All of our staff is required to be tested this week and tests returned to the designated lab through their designated delivery system no later than July 10th. BayView will start testing all staff on Wednesday, July 8th and continue through Thursday, July 9th. Testing will take place at the main entrance to the Samantha Wilson Care Center. If you are on your day off, please report to the facility on Wednesday or Thursday between 10:00 a.m. and 4:00 p.m. in order to complete the testing. You may not eat, drink or smoke 20 minutes prior to the testing. There will be a HIPAA consent form for the test and reporting results to administration made available for your signature at the time of your test. Any staff member who is unavailable to complete the test and the HIPAA authorization during this time frame will not be able to return to the facility. We regret that we were not given more notice and apologize for any inconvenience that this may cause, but we are operating under mandatory regulations for this testing program. Interim Update on Resident and Employee

COVID-19 Testing – June 29, 2020

The Florida National Guard conducted a Florida Department of Health mandatory Covid-19 test to all employees and any residents who gave their consent on Wednesday, June 24. As of June 29,   132 of the 169 test results for staff members have been received; 17 test results of the 75 residents who gave consent at the Samantha Wilson Care Center and 35 test results of the 57 residents at the Pavilion Assisted Living who gave their consent test have been received. All test results received to date were negative.

We are appreciative to the staff in their continued dedication in maintaining a safe environment for our residents and each other.

We were notified today one employee at the Samantha Wilson Care Center has tested positive for Covid-19. The test was administered on June 21st and this employee has not been in the building after June 15th. On June 16th this employee was isolated due to a dry cough. As is the case with all BayView employees, this employee was wearing a mask at all times while at work and worked on the first floor only. All BayView contacts with this employee were traced, and we have put in place enhanced respiratory monitoring and temperature and oxygen saturation checks every four hours. We continue to watch this matter carefully.

Dear Valued Residents, Family Members and Employees:

At approximately 2:00 this afternoon, the Florida Department of Health notified us that they going to provide the COVID-19 testing to all residents and staff at the Samantha Wilson Care Center and the Pavilion Assisted Living on Wednesday, June 24th starting at approximately 8:00 a.m. The testing program will be administered by one of their representatives. It is mandatory that staff be tested and, testing is voluntary for any resident who provide their consent. All residents and responsible parties will be notified to ensure consent and answer any questions. Additional details will be provided as to the when and where the staff should report and the plan that will be made available to administer tests to all residents.

Even though the new testing program is being put in place for all skilled nursing and assisted living facilities, there is no indication at this time for reopening long term care facilities and assisted living facilities to visitors.

Effective July 7, 2020 the Florida Agency for Health Care Administration (AHCA) will be requiring mandatory testing for the COVID 19 to all staff every two weeks. Details are pending regarding the type of tests to be used and how the tests will be distributed.

BayView is pleased to announce base line antibody testing will become available for all BayView staff. Testing kits have been ordered and should be available next week.

We are pleased to report there are no new COVID 19 cases at either at the Samantha Wilson Care Center or the Pavilion Assisted Living at BayView. BayView continues to follow enhanced CDC guidelines for infection control and prevention, which include screening, monitoring, testing, tracing in our system of care and isolation with support. Currently we have one employee who reported a cough and test results are pending. The second employee was non-systematic but exposed to someone who tested positive for the COVID 19. The first test result tested negative and a second test is pending. The third employee reported intestinal and respiratory systems and tested negative for the first test and is scheduling a second test. All three employees are isolating.

We continue to express our deep appreciation to our devoted staff for their hard work and dedication in providing great care to our residents and practicing strong safety measures to keep them as well as each other safe during this pandemic.

New notification regulations require us to “inform residents, their representatives, and families…following the occurrence of: a single confirmed infection of COVID-19; or three or more residents or staff with new-onset of respiratory symptoms that occur within 72 hours of each other.” There have been no new confirmed infections of COVID-19 at this time; however, three staff members did report new-onset of respiratory symptoms within 72 hours of each other.

1. The first staff member reported the new-onset of respiratory symptoms starting Saturday evening, May 2nd and continues to be in isolation. This staff member tested negative for COVID-19 on April 26th and has not been at the facility since April 30th. A second COVID-19 test result confirmed no COVID-19 infection.

2. The second staff member reported a new-onset of respiratory symptoms on May 3rd and is isolating at home. The result of a COVID-19 test is pending for this staff member.

3. The third staff member reported a new-onset of respiratory symptoms on May 5th and is isolating at home. This employee did complete a COVID-19 test today and results are pending.

BayView continues to emphasize the diligent reporting of any symptoms, screenings, enhanced monitoring, tracing contacts in our system of care and isolation with support. All staff members are screened and temperatures taken before entering the facility and continue to wear facemasks at all times. Residents are monitored for any signs or symptoms of COVID-19. Resident respiratory evaluations are completed daily and twice daily for those at heightened risk. Residents are encouraged to wear facemasks when out of their rooms or during prolonged care even when in their rooms.

BayView Healthcare Employee COVID-10 Testing Notice Scheduled for Sunday, April 26th – 7:30 a.m.

At approximately 10:30 a.m. this morning, the Florida National Guard notified BayView Administration that they will move forward to provide the COVID-19 testing to all employees on Sunday April 26th – 7:30 a.m.

All Bayview Employees are to report to the BayView campus on: Sunday, April 26th – by 7:15 a.m. – The National Guard will start the process at 7:30 a.m. Please meet at the main entrance to the Samantha Wilson Care Center. A registration table to sign in and clock in will be available. You need not enter the facility. If it is your scheduled day off, you still need to report to work and will be paid for your time.

Update on Resident and Employee COVID-19 Testing

This morning at 8:15 a.m., the Florida Department of Health arrived to the BayView campus and announced they were going to provide the COVID-19 testing to the Samantha Wilson Care Center and the Pavilion Assisted Living residents today. The test program that was to have been provided by the Florida National Guard, as we reported earlier during the week, had been withdrawn.

At approximately 4:20 p.m. this afternoon, the Department of Health was successful in completing the tests to all residents who provided consent at both the Samantha Wilson Care Center and the Pavilion. All but two residents provided consent to the test. Results will not be available most likely for a few days. It will be necessary to schedule the COVID-19 test to all employees at a later date.

We want to extend praise and appreciation to our staff in their efforts and dedication in making today’s event happen. We will continue to keep our community informed of updates throughout the COVID-19.Thank you.

BayView Healthcare is pleased to announce the Florida National Guard has offered to provide the COVID-19 test to all BayView residents and employees. This event is anticipated to be scheduled one day this week (April 21 through the 24th). This is an added safeguard measure to keep our population safe.  Residents and their responsible parties and BayView employees will be notified as soon as the date and details are made available. All employees will be required to have the COVID-19 test.  Details will be forthcoming through Paylocity, text messaging, BayView website, Facebook and communication by phone. We do apologize in advance for this short notice.

We are very grateful to the Florida National Guard who is providing this valuable service to our community.

BayView is dedicated in providing the best and safest environment for the protection of our residents and staff.   We are happy to report we have had no known COVID-19 virus incidents in the Samantha Wilson Care Center or The Pavilion Assisted Living.  The staff continue to provide the best possible care and we remain diligent in addressing CDC Guidelines as well as other healthcare resources to prevent the spread of the COVID-19 virus.

Effective March 18, 2020 the facilities are no longer accepting personal items or food from anyone from the outside of our complex into the facilities.  All resident personal laundry will be managed by inhouse staff.  These measures are recommended by the CDC Guidelines to further prevent the spread of the COVID-19 virus.

We are continuing to follow the CDC Guidelines for preventing the spread of the COVID-19 virus.  This includes the following updates in addition to the items posted on March 13th below:

Effective March 17, 2020 we will discontinue serving meals in the dining rooms.

All meals will be delivered to resident rooms until further notice.

All screened and approved visitors/vendors will be required to where masks when inside the building.

We know you may be concerned about the spread of the COVID-19 (the novel coronavirus) being reported in the media and how it may impact the residents and staff at BayView Healthcare. We remain committed to ensure that our residents are cared for in the safest and healthiest environment.

Effective Friday, March 13, 2020 at 5:00 p.m., all visitations to the Samantha Wilson Care Center and Pavilion Assisted Living have been suspended until further notice. This will apply to all family members, friends and volunteers.

All staff members will continue to follow the check-in screenings already in place.

Essential health care providers such as home health nurse and Hospice will be considered only if essential and allowed to enter the facility if they have non-systematic symptoms. Please contact Mr. David Wright at 904-315-7047 for any questions if you are considered to be a special provider at the Samantha Wilson Care Center or Ms. Theresa Ortiz, Administrator at the Pavilion Assisted Living at 904-755-6498.

End of life visitations will be considered. Please contact David Wright, RN, Director of Nursing at 904-315-7047 for any information.

We understand that connecting with family members is important and vital to their care. We suggest you remain in communication by use of telephone and/or email.

In addition, we will host a general meeting with any family member that would like to attend to address any of your questions or concerns regarding the COVID-19 processes in place at BayView Healthcare on the following dates:

Saturday, March 14th – 3:00 p.m. – BayView Courtyard or Monday, March 16th – 5:30 p.m. – BayView Courtyard

Please enter the courtyard through the gate located between the Samantha Wilson Care Center and the Pavilion Assisted Living.
