The St. Johns County Welfare Federation dba BayView Healthcare, is a 501(c)3 charitable and health care provider founded in 1920 to “take care of the sick and indigent person in the City of St. Augustine and the county of St. Johns.” BayView Healthcare has a long-standing history of providing long term care of the sick and frail, elderly and the indigent residents of the community. We offer a comprehensive umbrella of healthcare services. These healthcare services allow residents to age in place with the option of selecting the combination of services that best fit their needs now and in the future. Incorporating our professional staff, as liaisons between family and the healthcare needs of each client, we offer security and 24-hour availability to reduce stress and costs to the clients.

This is a patient centered program with the resident’s needs as the driving force. BayView Healthcare has all the programs to offer a full continuum of care (except acute care hospital service). The Continuum of Care offered through the programs of BayView Healthcare provides individualized care to elderly, homebound and chronically ill individuals based on:

• Quarterly care plans
• Daily assessments
• Change of status care planning
• Physician consultation as needed
• Monthly interdisciplinary quality improvement risk management meetings
• Internal financial controls and audits
• Dietary evaluations and nutritional consultations
•Socials service and case management across the healthcare continuum
• Physical, occupational, and speech therapy evaluations and recommended treatments

The important factor to understand is that our Continuum of Care is NOT A ONE WAY STREET and the ability to offer multi-level services that allows our resident to move through the system of healthcare on a individualized managed basis while striving to do so in the most appropriate and cost-effective way.

Our organization gives a “100% promise” to all donors and contributors. We spend 100% of all contributions on direct program costs. No funds are used to underwrite administrative overhead or executive salaries. With healthcare costs rising, grant funding decreasing, healthcare acuity levels rising and cost-cutting in Medicare and Medicaid, our agency manages with more needs with less money. Productivity levels have increased in the rehab and home health programs, and this has allowed us not to cut our levels of service.
